Urgency & Urge Incontinence

The intense feeling of having to rush, and sometimes not making it in time.

Urgency is the feeling; urge incontinence is the leak that happens before reaching the toilet. It is most often linked to an overactive bladder.

  • Key-in-the-door urgency A conditioned reflex: the need arises the moment you get home. The brain has associated coming home with releasing the bladder, a little too early.
  • The sound of running water Hearing water run triggers premature bladder contractions in the same way. These reflexes can be retrained.

Definition

Urge incontinence is a leak preceded by a sudden, irresistible urge. It often occurs in triggering situations: the sound of water, cold, arriving at the front door.

Causes

The same mechanisms as overactive bladder: uncontrolled bladder contractions, sometimes maintained by conditioning.

Diagnosis

  • Voiding diary noting the triggering situations.
  • Urodynamic assessment and cystoscopy as needed.

Treatments

Bladder training (gradually delaying urination, distraction techniques) and avoiding irritants (caffeine, alcohol) form the basis. Medicines can complement. Results build over a few weeks.
